Purpose


The Workplace Hazard Report Form is used to report hazards that have the potential to cause injury, illness or damage within the workplace. Prompt reporting allows hazards to be assessed and controlled before an incident occurs, supporting a safe working environment for employees, patients, contractors and visitors.



Management is responsible for maintaining the Workplace Hazard Report Form. Completed forms are stored within the organisation's governance records and are reviewed periodically, following reported hazards where appropriate, and as part of workplace health and safety review activities. All employees are responsible for reporting identified workplace hazards and completing this form as soon as practicable.


When is this form used?


This form should be completed whenever a workplace hazard is identified, regardless of whether an incident has occurred.


Examples include:

  • Slip, trip or fall hazards

  • Damaged or faulty equipment

  • Unsafe work practices

  • Manual handling hazards

  • Electrical hazards

  • Fire safety concerns

  • Biological or chemical hazards

  • Psychosocial hazards

  • Environmental hazards

  • Any other condition that may pose a risk to health and safety


Hazards should be reported as soon as practicable after they are identified.



Completing the Form


Provide as much information as possible to assist with hazard assessment and risk management.

Include:

  • The location of the hazard

  • A description of the hazard

  • Any immediate actions taken

  • Whether the hazard presents an immediate risk to health or safety

  • Any suggested control measures, where appropriate


Management will assess the reported hazard, determine appropriate control measures and monitor completion of any required actions.
