Purpose
The Patient Incident Form is used to document incidents involving patients that occur during the delivery of services at Bodyfocus Wellness Centre. Recording patient incidents supports appropriate clinical follow-up, investigation, risk management and continuous improvement, helping to maintain the safety and quality of care provided.
Management is responsible for maintaining Patient Incident Forms. Completed forms are stored within PracSuite and reviewed periodically, following significant patient incidents where appropriate, and as part of incident management and continuous improvement activities. All employees are responsible for promptly reporting patient incidents to the appropriate clinician or manager. Patient Incident Forms should be completed as soon as practicable following the incident.
When is this form used?
This form should be completed whenever an incident involving a patient occurs during the delivery of services, regardless of whether injury or harm occurred.
Examples include:
Patient falls
Injury during treatment
Adverse events
Medical emergencies
Unexpected deterioration in a patient's condition
Behavioural incidents affecting patient safety
Equipment failures affecting patient care
Medication or documentation errors
Any other event that results in, or has the potential to result in, harm to a patient
Where immediate clinical care or emergency assistance is required, this should always take priority over completing the Patient Incident Form.
Completing the Form
Patient Incident Forms are completed directly within PracSuite.
Provide clear, factual information regarding the incident, including:
The date, time and location of the incident
The people involved
A factual description of the incident
Any injury or harm sustained
Immediate actions taken
Witnesses, where applicable
Follow-up actions or recommendations
Patient Incident Forms form part of the patient's clinical record and should be completed objectively, accurately and as soon as practicable following the incident.
Management will review reported incidents, determine whether further investigation is required and implement any appropriate corrective actions.
