Purpose
The Incident Register records workplace and patient incidents reported to Bodyfocus Wellness Centre. It provides a central record of incidents, investigations, corrective actions and outcomes, supporting patient safety, workplace safety, legislative compliance and continuous improvement.
The register assists management to monitor incident trends, ensure appropriate follow-up and identify opportunities to reduce the likelihood of future incidents.
Management is responsible for maintaining the Incident Register. The register is stored within the organisation's governance records and is reviewed periodically, following significant incidents where appropriate, and as part of management review activities.
When is this register used?
A new entry should be created whenever an incident is reported using a Bodyfocus Incident Report Form or Patient Incident Report.
Incidents may include:
Patient incidents
Employee injuries
Near misses
Property damage
Security incidents
Privacy breaches
Aggressive or abusive behaviour
Other reportable workplace events
Each incident should be investigated as appropriate and recorded until all required actions have been completed.
Register Fields
Incident Type
Select the category that best describes the incident.
Incident Type | Description |
Patient Incident | An incident involving a patient during the delivery of services. |
Workplace Incident | An incident involving an employee, contractor, student or visitor. |
Near Miss | An event that had the potential to cause harm but did not result in injury or damage. |
Property Damage | Damage to buildings, equipment or other organisational property. |
Privacy or Information Security | Loss, unauthorised disclosure or inappropriate access to confidential information. |
Security | Theft, vandalism, threatening behaviour or other security-related incidents. |
Other | Any incident that does not fall within the above categories. |
Severity
Record the actual outcome of the incident.
Minor
Moderate
Major
Critical
Severity should reflect the outcome of the incident, not what might have happened.
Investigation Required
Select whether a formal investigation is required.

No
Incidents involving serious injury, repeated occurrences, safeguarding concerns or significant organisational risk will generally require investigation.
Status
Status | Description |
Open | The incident has been recorded and is awaiting review or action. |
Under Investigation | The incident is currently being investigated. |
Action in Progress | Corrective actions have been identified and are being implemented. |
Closed | Investigation and any required actions have been completed. |
Corrective Actions
Record any actions implemented to reduce the likelihood of a similar incident occurring again.
Examples include:
Staff education or training
Procedure updates
Environmental modifications
Equipment maintenance or replacement
Additional supervision
Policy review
Responsibility
Management is responsible for maintaining the Incident Register, ensuring incidents are investigated where required and monitoring completion of corrective actions. All employees are responsible for reporting incidents promptly in accordance with organisational procedures.
