Bodyfocus Wellness Centre welcomes feedback and is committed to resolving concerns fairly, respectfully, and promptly.
If you would like to provide feedback or make a complaint, please enter your details below. A secure link to access our Patient Feedback and Complaint Form will then be sent to you.
This process helps ensure complaints are documented accurately, investigated appropriately, and stored securely within your patient record.
Feedback and complaints may also be raised in person, by phone, by email, or through a representative. Where a formal complaint is made, a Patient Feedback and Complaint Form will be provided to support the review process.
If you are not satisfied with the outcome of your complaint, you may contact the NDIS Quality and Safeguards Commission or another relevant external agency.
